Effective Jan. 1, 2023, the partnership will enhance patient and provider solutions to PathfinderHealth primary care procedures in northern Arizona and help PathfinderHealth, a leading Clinically Integrated Network and Accountable Care Company, navigate the transformation to value-based health care endeavors.
After performing an extensive national search, Equality Health was chosen to partner with PathfinderHealth to increase provider happiness and improve patient care consistently and health consequences. Equality Health also will provide PathfinderHealth with further resources to manage the total revenue of care via a localized whole-person, cultural care model. Equality Health’s amazing cultural care model and the companies’ shared task of providing quality care to underserved individuals forged the connection.
